OK it created the guest then:

Xen error. Add libc6-xen to the system and they will go away, or look
at the ld.conf file.

That looks like /etc/inittab is incorrect, or not present. Is that
the case?
> /usr/lib/xen-tools/etch.d/20-setup-apt: line 88: chroot: command not found
If PATH contains /usr/sbin then this shouldn't occur. I'd guess this
is not relevant, and ignore your mail from here down.
